Health / Vision / Dental Insurance
|
After completing two months of continuous full-time employment. If elected, coverage is effective the first day of the month following the completion of the two months.

Life Insurance
|
After completing two months of continuous full-time employment. Basic Group Life/AD&D is provided at no cost for 2x annual salary maximum of $100,000. Coverage is effective the first day of the month following the completion of the two months.
|
|
Long Term Disability
|
After completing two months of continuous full-time employment. Coverage is required by employee for 66.67% of monthly salary with a maximum of $6,000. Coverage is effective the first day of the month following the completion of the two months.

|
Voluntary Life Insurance
|
After completing two months of continuous full-time employment. If elected, coverage is effective the first day of the month following the completion of the two months.
|
|
Health Care Expense Account
|
After completing two months of continuous full-time employment. University Title Company will put $500 in the employee's account if the employee is enrolled in the UTC group health insurance plan. The health care expense account is only available if you are enrolled in UTC's group health insurance and is a tax-free account.

|
Dependent Care Expense Account
|
After completing two months of continuous full-time employment. University Title Company provides this account for each employee who has childcare expenses for children under the age of 13 the opportunity to set aside in a tax-free account up to $5000 for dependent care expense reimbursement if the employee is single or married filing jointly.

|
Cancer Insurance
|
After completing two months of continuous full-time employment. If elected, coverage is effective the first day of the month following the completion of the two months.
|
|
401 (K) Profit Sharing
|
After completing one year of continuous employment, with at least 1,000 hours worked, and at least 21 years of age.
|
|
Sick Leave
|
After completing two months of continuous full-time employment. An employee earns one day or eight hours of paid sick leave per month after the two month introductory period for a total of five days of forty hours of paid sick leave.
|
|
Vacation Leave
|
After completing six months of continuous full-time employment. An employee will receive five days or forty hours of paid vacation leave.
|
|
Extended Medical Leave Bank
|
After completing one year of continuous employment. For every year of service to University Title Company, an employee will receive 5 days or 40 hours in their extended leave bank.

Full Time Employee Benefits
|
|
|
University Title Company sponsors a comprehensive insurance plan from Great West Health Care.
|
|
As a UTC full time employee working a schedule of forty (40) hours or more each week, you become
|
|
eligible to enroll for group insurance once you have completed two (2) months of continuous full time
|
|
employment.
|
|
|
|
|
Part Time Employee Benefits
|
|
|
If you work less than forty (40) hours per week, you are not eligible for these benefits; however, if you
|
|
change to a schedule where you work forty (40) hours a week, you will become eligible to enroll in
|
|
group insurance once you have completed two (2) months of employment at the full-time schedule.

|

Premiums
|
|
|
UTC will share the cost of premiums for the above insurance. When you are notified of your opportunity
|
|
to enroll, cost sharing will be described in greater detail. If you elect coverage, your share of premium
|
|
contributions will be through payroll deduction.
|
|
|
|
|

Paid Leave
|
|

An employee with 3-5 years of service earns 2.0 days or 16 hours per month (8 hours per pay period)
|
|
of paid leave time for the first 10 months worked for a total of 20 days or 160 hours per year.
|
|
|
|
|
An employee with 6-9 years of service earns 2.5 days or 20 hours per month (10 hours per pay period)
|
|
of paid leave time for the first 10 months worked for a total of 25 days or 200 hours per year.
|
|
|
|
|
An employee with 10 years of service or more earns 3.0 days or 24 hours per month (12 hours per pay
|
|
period) of paid leave time for the first 10 months worked for a total of 30 days or 240 hours per year.
